Access to the Physician Portal from outside of the UH network was disabled on January 31, 2019. If you are a UH employee or work for an affiliated clinical facility, please access MyApps prior to accessing the Physician Portal. Access to the Community Record is now available from outside of the UH network via the link to the left.

Requesting Access

UH Providers and Staff (includes affiliates and partners):
  • To request access to the Community Record or the Physician Portal, please contact your Manager, designee or UH sponsor to complete and submit an eSecurity form on your behalf.
  • Access to CliniSync can be done through the links in UH Acute and Ambulatory EMRs.
Non-UH Providers and Staff:
  • Physician Portal access for non-UH, non-affiliated providers and staff is no longer being granted as of June 1, 2018.
  • To request access to the Community Record, please contact your UH sponsor to complete and submit an eSecurity form on your behalf.
